Tous les ans au Canada, environ une centaine de patients p\u00e9diatriques re\u00e7oivent un diagnostic de m\u00e9dulloblastome, la tumeur la plus r\u00e9pandue chez l\u2019enfant.<\/p>\n<p>Jusqu\u2019\u00e0 maintenant, poupons et enfants sont trait\u00e9s de la m\u00eame mani\u00e8re, au moyen de la chirurgie associ\u00e9e \u00e0 la radioth\u00e9rapie du cerveau entier et \u00e0 une chimioth\u00e9rapie intensive. Malgr\u00e9 ce traitement, entre 30% et 40% des enfants atteints meurent en raison de la tumeur primitive. Parmi les survivants, entre 30% et 40% d\u00e9veloppent une tumeur r\u00e9cidivante qui entra\u00eene in\u00e9vitablement la mort dans la quasi-totalit\u00e9 des cas. Beaucoup des autres survivants sont atteints de d\u00e9ficiences neurologiques, intellectuelles et physiques, des s\u00e9quelles laiss\u00e9es par le traitement sur leurs cerveaux immatures.<\/p>\n<p>D\u00e9termin\u00e9s \u00e0 changer la fa\u00e7on dont ces enfants sont soign\u00e9s, des chercheurs du BC Cancer Agency Genome Sciences Centre se sont associ\u00e9s \u00e0 des cliniciens et \u00e0 des chercheurs de l\u2019H\u00f4pital pour enfants malades de Toronto et de 46 centres de canc\u00e9rologie partout dans le monde pour cr\u00e9er un consortium international appel\u00e9 MAGIC, le Medulloblastoma Advanced Genomics International Consortium (consortium international de la g\u00e9nomique avanc\u00e9e du m\u00e9dulloblastome). Ensemble, ils ont recueilli plus de 1 000 \u00e9chantillons de m\u00e9dulloblastomes p\u00e9diatriques.<\/p>\n<p>\u00c0 l\u2019aide d\u2019une technologie de pointe, les chercheurs canadiens ont analys\u00e9 l\u2019ensemble des g\u00e9nomes de tumeurs primitives et de tumeurs r\u00e9cidivantes. Ils ont confirm\u00e9 non seulement l\u2019existence de quatre sous-groupes mol\u00e9culaires distincts de m\u00e9dulloblastomes, mais \u00e9galement de sous-ensembles cliniquement significatifs. Ils ont aussi d\u00e9couvert que le profil g\u00e9n\u00e9tique d\u2019une tumeur r\u00e9cidivante n\u2019a presque rien en commun avec la tumeur primitive d\u2019origine. \u00ab Voil\u00e0 pourquoi les traitements n\u2019ont pas d\u2019effet sur les tumeurs r\u00e9cidivantes \u00bb, explique le Dr\u00a0Michael Taylor, neurochirurgien p\u00e9diatrique et chercheur \u00e0 l\u2019H\u00f4pital pour enfants.<\/p>\n<p>La classification des tumeurs permettra aux cliniciens de diagnostiquer et de traiter des patients p\u00e9diatriques atteints de m\u00e9dulloblastomes avec une plus grande pr\u00e9cision \u2013 par exemple, diminuer la radioth\u00e9rapie pour les enfants atteints d\u2019une tumeur \u00e0 faible risque pour pr\u00e9server leur qualit\u00e9 de vie, tout en maintenant les taux de gu\u00e9rison.<\/p>\n<p>Gr\u00e2ce \u00e0 la d\u00e9termination de cibles mol\u00e9culaires sp\u00e9cifiques d\u2019intervention, la recherche pourra aussi contribuer \u00e0 l\u2019am\u00e9lioration des essais cliniques et \u00e0 la mise au point de nouveaux m\u00e9dicaments th\u00e9rapeutiques adapt\u00e9s aux patients.<\/p>\n<p>Le Dr\u00a0Taylor codirige le projet MAGIC avec le Dr\u00a0Marco Marra, directeur du Genomics Innovation Centre au BC Cancer Agency Genome Sciences Centre (l\u2019un des 10 p\u00f4les du R\u00e9seau d\u2019innovation g\u00e9nomique financ\u00e9s par G\u00e9nome Canada au pays) et le Dr\u00a0David Malkin, oncologue principal de l\u2019H\u00f4pital pour enfants malades.<\/p>\n<p>Comprendre les diff\u00e9rences dans la composition g\u00e9n\u00e9tique de ces tumeurs et l\u2019influence de ces diff\u00e9rences sur la r\u00e9ponse au traitement r\u00e9duira le lourd fardeau de souffrance et de mis\u00e8re des familles dont les enfants ne re\u00e7oivent peut-\u00eatre pas le traitement le plus appropri\u00e9.
