Ils l\u2019emportent rapidement dor\u00e9navant sur la lign\u00e9e ancestrale end\u00e9mique dans diverses r\u00e9gions du monde en raison de leur transmissibilit\u00e9 nettement sup\u00e9rieure (Coutinho et coll. 2021). Le fait qu\u2019ils \u00e9chappent \u00e0 l\u2019immunit\u00e9 naturelle (p.\u00a0ex., des personnes pr\u00e9c\u00e9demment infect\u00e9es par un variant ant\u00e9rieur sont r\u00e9infect\u00e9es par un variant pr\u00e9occupant) et \u00e0 l\u2019immunit\u00e9 induite par la vaccination pr\u00e9occupe de plus en plus. En Ontario, les variants pr\u00e9occupants sont associ\u00e9s \u00e0 une augmentation de 63\u00a0% du risque d\u2019hospitalisation, de 103\u00a0% du risque d\u2019admission dans les unit\u00e9s de soins intensifs (USI) et de 56\u00a0% du risque de d\u00e9c\u00e8s d\u00fb \u00e0 la COVID-19 (Ontario Science Advisory Table 2021). Les approches utilis\u00e9es actuellement pour identifier les variants pr\u00e9occupants sont bas\u00e9es sur l\u2019analyse quantitative de la r\u00e9action en cha\u00eene de la polym\u00e9rase en temps r\u00e9el (qRT-PCR) et le s\u00e9quen\u00e7age du g\u00e9nome d\u2019\u00e9chantillons cliniques. Le taux d\u2019attaque des variants pr\u00e9occupants est si \u00e9lev\u00e9 que la lenteur et les biais associ\u00e9s aux tests cliniques traditionnels emp\u00eachent de signaler de mani\u00e8re pr\u00e9cise et opportune l\u2019incidence des variants pr\u00e9occupants aux services de sant\u00e9 publique. Il faut des mesures additionnelles \u00e0 l\u2019\u00e9chelle de la population, ind\u00e9pendantes des donn\u00e9es cliniques, pour fournir des donn\u00e9es exploitables aux services de sant\u00e9 publique. La d\u00e9tection des variants pr\u00e9occupants dans les eaux us\u00e9es, \u00e0 l\u2019aide des m\u00e9thodes de qRT-PCR et de s\u00e9quen\u00e7age, est une source d\u2019information inexploit\u00e9e qui peut r\u00e9pondre \u00e0 ce besoin. En outre, l\u2019identification des variants pr\u00e9occupants dans les eaux us\u00e9es renseignera sur leur pr\u00e9sence dans les collectivit\u00e9s, puisque les \u00e9chantillons d\u2019eaux us\u00e9es contiennent des d\u00e9chets f\u00e9caux provenant de nombreuses personnes et que de multiples variants pourraient \u00eatre trouv\u00e9s dans chaque \u00e9chantillon d\u2019eaux us\u00e9es.<\/p>\n<p>Un programme pilote cibl\u00e9 de surveillance des variants pr\u00e9occupants dans les eaux us\u00e9es de l\u2019Ontario fournira des donn\u00e9es pr\u00e9cieuses pour la d\u00e9tection h\u00e2tive, le suivi des tendances et l\u2019\u00e9valuation des risques pour les collectivit\u00e9s. Cette proposition vise \u00e0 mobiliser un groupe de base de chercheurs qui \u00e9laboreront et mettront en \u0153uvre \u00e0 l\u2019\u00e9chelle de l\u2019Ontario un programme de surveillance des variants pr\u00e9occupants du SRAS-CoV-2 dans les eaux us\u00e9es, qui consiste en une analyse par qRT-PCR des eaux us\u00e9es pour y d\u00e9tecter les variants pr\u00e9occupants, suivie d\u2019un s\u00e9quen\u00e7age du g\u00e9nome des \u00e9chantillons positifs \u00e0 la qRT-PCR afin d\u2019assurer un suivi approfondi des variants pr\u00e9occupants connus et \u00e9mergents. Le programme pilote mobilisera les \u00e9quipes ontariennes qui connaissent d\u00e9j\u00e0 l\u2019application des tests qRT-PCR afin de surveiller les variants pr\u00e9occupants du SRAS-CoV-2 dans toute la province. Les \u00e9chantillons seront analys\u00e9s par qRT-PCR dans toutes les r\u00e9gions de la province toutes les deux semaines, et les \u00e9chantillons de chaque r\u00e9gion seront \u00e9galement analys\u00e9s par s\u00e9quen\u00e7age pour d\u00e9terminer la pr\u00e9sence de variants pr\u00e9occupants. Si les donn\u00e9es de s\u00e9quen\u00e7age r\u00e9v\u00e8lent l\u2019existence d\u2019un nouveau variant pr\u00e9occupant pour lequel il n\u2019existe pas d\u2019analyse par qRT-PCR, une nouvelle analyse sera rapidement mise au point et distribu\u00e9e aux membres de l\u2019\u00e9quipe charg\u00e9e de la surveillance par qRT-PCR. La surveillance des variants pr\u00e9occupants dans les eaux us\u00e9es est une approche compl\u00e9mentaire de la surveillance clinique. Les partenariats indispensables actuels avec les services de sant\u00e9 publique municipaux, provinciaux et f\u00e9d\u00e9raux sont mis \u00e0 profit pour assurer la diffusion rapide des donn\u00e9es sur les variants pr\u00e9occupants du SRAS-CoV-2 dans les eaux us\u00e9es.
